A 500-bed multi-speciality hospital in India is a project of several hundred crore rupees that takes three to four years to reach its first patient, and then loses money for a while before it earns. Aster DM Healthcare announced a 500-bed hospital in Bengaluru in August 2025 with a ₹580 crore investment, on a leased building, for opening in the second half of FY 2028-29.
Announced projects from the larger chains work out to about ₹1.2 crore to ₹2 crore per bed, so 500 beds means roughly ₹580 crore to ₹1,000 crore, with land inside or outside that figure depending on the project. Three decisions move the number most: how you hold the land and building, how many beds you open on day one, and which patients fill them.
What does a 500-bed hospital cost, and why do the published numbers differ?
Divided by beds, announced investments sit between ₹1.16 crore and ₹2 crore per bed. The spread comes from what each figure includes.
| Project | Beds | Announced investment | Per bed | What to note |
|---|---|---|---|---|
| Aster DM Healthcare, Bengaluru | 500 | ₹580 crore | ₹1.16 crore | Leased built-to-suit building, about 5 lakh sq ft |
| Narayana Health expansion plan | 2,000 | ₹3,000 crore | ₹1.5 crore | About 20% spent, mostly on land |
| Max Healthcare expansion plan | 3,700 | ₹6,000 crore | ₹1.62 crore | New hospitals and expansions mixed |
| AIG Hospitals, Visakhapatnam | 1,000 | ₹2,000 crore | ₹2 crore | Includes a nursing college |

The NPCS project report profiles for the nearest matches sit lower, so read them as the bottom of the range.
| NPCS report | Capacity | Plant and machinery | Total capital investment | Return | Break-even |
|---|---|---|---|---|---|
| Medical College & Hospital | 500 beds plus college | ₹17.28 crore | ₹79.57 crore | 40.00% | 39.00% |
| Hospital | 150 beds | ₹14.86 crore | ₹62.03 crore | 26.33% | 40.12% |
The profiles state no cost year, so ask for current-year costs before showing them to a lender. Whatever figure you use, check that it includes land, building, equipment, interiors, pre-opening losses and working capital.
Buy the land, lease a built-to-suit building, or take a PPP site?
| Route | Upfront cash | What to watch |
|---|---|---|
| Buy and build | Highest; Narayana's first 20% went mostly on land | The plot must carry about 5 lakh sq ft of floor area plus room to expand |
| Built-to-suit lease | Lower; the landlord builds to your specification, as in Aster's 30-year lease | Rent escalation, and who pays for added floors |
| Public-private partnership (PPP) on government land | Land cost moves to the state | Free-bed duties: NITI Aayog's 2021 district hospital model reserves free inpatient care for 300 beds plus 20% of extra beds |
With no site yet, price the built-to-suit route first. It turns a construction gamble into a rent you can model.
How much does a 500-bed hospital earn, and how long does it lose money?
ARPOB is revenue per occupied bed per day. EBITDA is operating profit before interest, depreciation and tax.
| Hospital | Beds | What the latest results show |
|---|---|---|
| Max Healthcare network, April to June 2026 | 5,379 | Occupancy 75%, ARPOB ₹81,900, EBITDA margin 24.8% |
| Max's Bhubaneswar hospital after acquisition, same quarter | 250 | Occupancy 50%, ARPOB ₹35,000, EBITDA about ₹2 crore on ₹19 crore revenue |
| Medanta, Noida, first year (FY26) | 382 | EBITDA loss of ₹78.3 crore on ₹90.6 crore revenue |
Medanta's Noida loss narrowed from ₹32 crore in the third quarter to ₹23.6 crore in the fourth, according to its 14 May 2026 release. The NPCS profiles model break-even at 39% to 40% of capacity. Test your plan at lower occupancy than that, and carry the start-up losses in the funding plan as a line of their own.
How to start a 500-bed multi-speciality hospital: which approvals come first?
Building clearances come first, radiation and fire approvals before the machines and stairs are fixed, and registration and waste authorisation before opening.
| Order | Approval | Issued by | What it means for you |
|---|---|---|---|
| 1 | Building plan and environmental clearance | Local authority; State Environment Impact Assessment Authority | Needed above 20,000 sq m built-up area. Aster's 5 lakh sq ft is about 46,000 sq m, so assume yours is too |
| 2 | Radiation layout approval | Atomic Energy Regulatory Board (AERB), e-LORA portal | Upload shielded room layouts before buying a CT scanner or cath lab; each needs an approved radiation safety officer |
| 3 | Fire safety certificate | Local fire authority | Checked against National Building Code 2016, Part 4: sprinklers, two exits per floor, 30 m travel distance |
| 4 | Clinical establishment registration | State health department | Provisional for 12 months, then permanent for five years; operating unregistered risks a penalty up to ₹50,000 for a first offence |
| 5 | Biomedical waste authorisation | State Pollution Control Board | No on-site treatment if a common facility is within 75 km; no storing untreated waste beyond 48 hours |
The central Clinical Establishments Act applies in the states and union territories that have adopted it. Elsewhere, ask your state health department which registration law applies.
NABH accreditation sits outside this list. The empanelment guidelines of the Ayushman Bharat PM-JAY insurance scheme do not treat it as a prerequisite, but accredited hospitals are fast-tracked, and the 6th edition of the standards applies to new applications from 1 January 2025.
Which services and schemes help fill 500 beds?
Pick the opening specialties by the referring doctors you can sign, the cost and approvals of their machines, and the mix of insured, cash and scheme patients.
| Patient source or support | What it gives you | What to check |
|---|---|---|
| PM-JAY empanelment | Ayushman Bharat patients. Empanelment is voluntary, and the state health agency decides, aiming for 30 working days | Certified hospitals earn higher package prices; cardiology, cancer care, neurosurgery and nephrology carry extra criteria |
| State hospital-investment policies | Some states offer capital subsidy or priority land; Odisha's Health Care Investment Promotion Policy is one example | Amounts and validity change, so read the current policy and the government schemes section |
| Medical tourism | The Union Budget 2026-27 announced five integrated medical hubs with private participation. Medical tourist arrivals passed 6 lakh in 2024, per the Economic Survey 2025-26 as summarised by KPMG | Eligibility rules for the hubs were not in the coverage; check the health ministry |
What should you do in the first 90 days?
- Price the land route: two purchase sites and one built-to-suit offer.
- Commission a feasibility study of referral doctors, insurer panels and competing beds in your catchment.
- Fix phase one. AIG opens 650 of its 1,000 beds first, and Max opened 202 of 400 at Max Smart. Build the shell and utilities for all 500 beds, but fit out and staff phase one only.
- Freeze the equipment list with AERB layouts attached.
- Build the funding plan with start-up losses included. Banks ask for a detailed project report, and startup funding options cover equity.
NPCS has prepared project reports since 1994, over 5,000 of them across more than 40 sectors. A hospital report covers the market survey, plant and machinery, land and building, manpower, project cost, working capital, break-even and cash flow, in the format banks usually ask for. It supports a lending decision but does not replace the demand study for your site.
To supply hospitals rather than own one, start from the pharma manufacturing ideas hub, the IV fluid plant guide or the hospital rubber sheet guide.
Frequently asked questions about a 500-bed hospital
Can a non-doctor open a hospital in India?
Yes. The Clinical Establishments Act defines an establishment by who sets it up and runs it, whether a company, trust, society or single doctor, and does not require the owner to be a doctor. It does require prescribed staff and facilities, and state rules differ, so check with your state health department.
How long does it take to build a 500-bed hospital?
Plan three to four years from site to first patient. Aster announced its Bengaluru hospital in August 2025 for the second half of FY 2028-29, and AIG's Visakhapatnam phase one, announced in September 2026, targets October 2028.
How much land does a 500-bed hospital need?
There is no fixed acreage. Take the built-up area (Aster's 500-bed project is about 5 lakh sq ft), divide it by the floor area ratio your authority allows, then add parking, fire access and room to expand.
Is NABH accreditation compulsory for a new hospital?
Not for PM-JAY empanelment, where the guidelines do not require it, though accredited hospitals are fast-tracked and certified ones earn higher package prices. The registration law sets its own minimum standards.
