To start a surgical methylated spirit manufacturing business, you set up a blending and bottling unit, not a distillery. You buy ready spirit, mix it with skin conditioners such as castor oil and methyl salicylate, test every batch and sell to hospitals, pharmacies and clinics.
Three permissions come before any machine is ordered: a drug manufacturing licence from your state drugs control office, an excise permit to buy and hold the spirit, and fire safety clearance for a store full of flammable liquid.
A large part of the money goes into stock, not machines. In the project report from Niir Project Consultancy Services (NPCS) for a 10,000 litre a day plant, working capital is ₹96 lakh against ₹69 lakh for plant and machinery, in a total investment of ₹179 lakh.
Which product are you making: surgical spirit or methylated spirit?
The two names are used loosely, but they lead to different approvals, so settle this first. Surgical spirit is a medicine for external use, which puts it under the Drugs and Cosmetics Act. The UK-approved label for one brand lists methyl salicylate at 0.5% v/v and ethanol at 90.25% v/v, with diethyl phthalate and castor oil, all made from industrial methylated spirit (IMS). IMS is ethanol made unfit to drink by a denaturant such as methanol. Indian pharmacy listings show the same four ingredients to Indian Pharmacopoeia standard.
| Surgical spirit | Industrial methylated spirit | |
|---|---|---|
| What it is | Ethanol about 90%, with methyl salicylate, diethyl phthalate, castor oil | Ethanol of 95% strength, denatured |
| Who buys it | Hospitals, pharmacies, clinics | Laboratories, cleaning and solvent users, surgical spirit makers |
| Main approval | Drug manufacturing licence, plus excise permit | State excise rules for denatured spirit |
Make surgical spirit if you can reach hospital and pharmacy buyers and are ready for the drug licence. Choose IMS only if industrial buyers are already lined up; that business sits closer to industrial alcohol manufacturing than to pharma.
How is surgical methylated spirit made?
The work is controlled blending, so quality comes down to measurement and records.
- Receive the spirit against your excise permit and check its strength and quantity before it enters the store.
- Measure the batch into a closed stainless steel tank.
- Add castor oil, diethyl phthalate and methyl salicylate, and stir with a flameproof agitator until uniform.
- Test a sample for ethanol strength and methyl salicylate content before release.
- Filter, fill, seal and label with batch number, expiry and the flammable and external use warnings.
- Hold the batch until the lab releases it, then store away from heat and dispatch.
Take additive quantities from the pharmacopoeial formula you register the product against. The label above fixes only ethanol and methyl salicylate.
Which machines does a spirit blending and filling unit need?
Every machine that touches spirit must be built for flammable liquid. The lab belongs in the licence file too, because drug authorities ask for machinery and quality control instrument lists with the application.
| Equipment | What it does | Check with the supplier |
|---|---|---|
| Stainless steel blending tank with flameproof stirrer | Mixes spirit and additives in closed batches | Volume markings, flameproof certificate for the motor |
| Spirit storage tanks | Hold permitted stock | Capacity within your excise and fire approvals |
| Flameproof pump and flow meter | Moves and measures spirit | Accuracy, leak-free joints |
| Filter | Removes particles before filling | Material that spirit does not attack |
| Filling, capping and labelling line | Fills, seals and codes each pack | Your pack sizes; the second NPCS report below assumes 1 litre, 5 litre and 20 litre packs |
| Quality control lab | Tests strength and additive content | Instruments match your application list |
Which licences does a surgical spirit plant need?
Five approvals apply, and the first two decide whether the project is possible. Rules differ by state, so this is the usual sequence as of October 2026.
| Approval | Issued by | What to know |
|---|---|---|
| 1. Excise permit for spirit | State excise department | A nine-judge Supreme Court bench held on 23 October 2024 that states can regulate industrial alcohol, so get your state's denatured spirit rules before choosing a plot. |
| 2. Drug manufacturing licence | State drugs licensing authority | In Goa, the drugs directorate takes the Form 24 application online with a site master file, premises plans, machinery and quality control instrument lists, named technical staff and the lease deed. A drugs inspector and a Central Drugs Standard Control Organisation official inspect against Schedule M. |
| 3. Fire safety clearance | Local fire service | For a store of flammable liquid. Ask before fixing the layout; changing a building costs more than changing a drawing. |
| 4. Petroleum Rules licence | Petroleum and Explosives Safety Organisation (PESO) | PESO's list of substances needing a licence includes ethyl alcohol. Ask your PESO circle office whether your storage needs one. |
| 5. GST and Udyam registration | GST portal, Ministry of MSME | The GST rate depends on classification, so confirm the code with your accountant. Udyam opens the MSME schemes. |
Schedule M, the good manufacturing practice rulebook for drug makers, was revised in December 2023, so design the premises to the revised version. Name your qualified technical staff before applying; the licence names them and any change must be reported.
What does it cost to start a surgical methylated spirit manufacturing business?
NPCS has two project reports on this product, built for different plant sizes and pack mixes. Read them as two worked examples, not a price list.
| Item | 10,000 litre a day report | Mixed-pack report |
|---|---|---|
| Capacity | 10,000 litres a day | 4,000 one-litre bottles, 480 five-litre bottles and 80 twenty-litre cans a day |
| Plant and machinery (₹ lakh) | 69 | 79 |
| Total capital investment (₹ lakh) | 179 | 265 |
| Return (%) | 45.19 | 27.53 |
| Break-even point (%) | 60.81 | 54.72 |
Working capital is the line to watch. In the first report it is ₹96 lakh, more than the machinery, which fits a business that must buy spirit before it sells product. Capacity, pack mix, spirit price and credit to hospital buyers all move the return and break-even point. Figures vary by location and technology, and a lender will want a report built for your site.
NPCS has prepared over 5,000 project reports across 40+ sectors since 1994, and these two come from that work.
What goes wrong in a spirit blending business?
- Permit mismatch: Excise officers track spirit bought, used and sold, so keep batch records that reconcile to the litre.
- Methanol and fire: The UK label warns that swallowing methylated spirit or breathing its vapour can cause methyl alcohol poisoning, with symptoms delayed by up to 48 hours, and calls the product flammable. Keep denatured spirit apart from finished goods and design for fire from day one.
- Cash tied up in stock: Spirit is paid for before it is sold, so a late hospital payment squeezes the next purchase.
What should you do first?
- Choose surgical spirit or IMS, and the pack sizes your buyers use.
- Get your state's denatured spirit rules and permit route before committing to a plot.
- Shortlist premises for a flameproof blending hall, a separate spirit store and a lab, and find a qualified technical person.
- File the drug licence and fire safety applications together, and ask PESO about storage.
- Compare this unit with other pharma manufacturing ideas, and read what a detailed project report contains before approaching a bank.
An NPCS project report on surgical spirit covers the manufacturing process with flow sheet, machinery with suppliers, raw material costs, manpower, project cost, working capital, cost of production and break-even, in the format banks and schemes such as PMEGP and Mudra typically ask for. It does not replace the state approvals.
Frequently asked questions
Is methylated spirit 100% alcohol?
No. Methylated spirit is ethanol with a denaturant added to make it unfit to drink, plus some water. The industrial methylated spirit in the UK-approved label above is 95% ethanol, and the surgical spirit made from it holds 90.25% v/v.
Can methylated spirit be used as a face cleanser?
No. Surgical spirit is labelled for preventing bed sores and hardening the skin of the feet, and may cause a burning sensation on damaged skin. Industrial methylated spirit is made for cleaning and solvent use, and swallowing it or breathing its vapour can cause methyl alcohol poisoning.
What should you not use methylated spirits on?
Not on broken skin, where it may burn, and not near a naked flame, because the label calls it flammable. Do not swallow it or breathe in its vapour. Methyl alcohol effects can appear up to 48 hours later, so anyone exposed needs medical advice even if they feel well.
How is methylated spirit obtained?
It starts as ethanol from fermented molasses or grain. Distillation makes strong rectified spirit, and a denaturant is added under excise control. A surgical spirit maker usually buys it, because making it needs a distillery, a far larger project covered in absolute alcohol manufacturing.
