What is a surgical spirit project report?
A surgical spirit project report is a detailed project report (DPR) for a unit that blends rectified spirit with small amounts of methanol, castor oil, diethyl phthalate and methyl salicylate, then fills it into bottles and cans. It sets out the process, licences, machinery, raw materials, project cost, five-year projections, DSCR and break-even.
Pharma entrepreneurs use it to plan the unit and apply for a term loan; banks use it to judge whether the plant can repay.
Also known as: surgical methylated spirit project report, denatured alcohol (surgical spirit) project report, surgical spirit business plan, DPR for surgical spirit manufacturing.
What a bank checks in a surgical spirit project report
A surgical spirit unit is a blending and filling plant, so its profit rests on the price of spirit, the cost of packs and how fast buyers pay. Lenders test these four numbers first. The values are the ones used in our sample report.
About half of every rupee of sales goes on spirit and additives. A report that shows far less is usually pricing spirit too low or sales too high.
Spirit is the one input that matters. Banks look for a test of what a rise in its permit price does to profit and DSCR (debt service coverage ratio).
Bottles, caps, labels and cartons cost more per litre in 100 ml packs than in 5 litre cans. The pack mix must match the packing cost.
Hospitals, distributors and government tenders rarely pay on delivery. Debtor days set the cash credit limit the unit needs.
Values are the assumptions in our illustrative sample. Your report uses figures for your capacity, spirit supplier and buyers.
How much does a surgical spirit plant cost in India?
Cost depends on daily output, how much of it goes into small bottles and whether you build your own bonded spirit store. The large tier is the plant modelled in our sample report; the other two are indicative ranges.

Surgical spirit manufacturing process
There is no distillation on site. The unit buys rectified spirit under an excise permit, denatures and blends it to the pharmacopoeia formula, and fills it. Every transfer is measured, because the excise department reconciles each litre of spirit received against the litres sold.
- Receiving spirit
Tankers arrive sealed under an excise transport pass; the spirit is gauged and its strength checked with an alcoholometer before it goes into locked tanks.
- Denaturing and blending
Measured spirit is pumped into a closed stainless steel vessel and mixed with methanol, castor oil, diethyl phthalate and methyl salicylate.
- Batch testing
The lab checks ethanol content, methanol level, specific gravity and clarity against the IP monograph before releasing the batch.
- Filtration
Cartridge filters remove fibres and haze so the liquid stays clear in transparent bottles.
- Filling and capping
Flameproof volumetric fillers fill 100 ml to 500 ml bottles and 5 litre cans, which are sealed with tamper-evident caps.
- Labelling and coding
Each pack carries the composition, batch number, manufacturing and expiry dates and a 'for external use only' warning.
- Storage and dispatch
Cartons wait in a ventilated flammable goods store and leave against both excise and sales records.
Raw materials for surgical spirit
Rectified spirit (95% ethanol) comes from molasses or grain distilleries and can be bought only against a state excise permit, so a second approved supplier is worth arranging early. Methanol must carry the BIS mark under the IS 517 Quality Control Order, in force from 3 August 2025. The additives are bought to IP grade.
| Raw material | Share of cost | Year 3 (₹ lakh) |
|---|---|---|
| Rectified spirit (ethanol 95%) | 87.0% | 522.76 |
| Castor oil IP | 4.5% | 27.04 |
| Diethyl phthalate IP | 4.0% | 24.03 |
| Methyl salicylate IP | 2.5% | 15.02 |
| Methanol (IS 517 grade) | 2.0% | 12.02 |
| Total | 100.0% | 600.87 |
Bottles, caps, labels and cartons are costed separately as packing material, ₹102.67 lakh in Year 3 of the sample.
Machinery for a surgical spirit plant
Spirit vapour can catch fire at room temperature, so motors, panels and lighting in the blending and filling areas must be flameproof. In the sample, spirit storage is the largest machinery item, followed by the filling machine.
| Machine | What it does | Sample cost (₹ lakh) |
|---|---|---|
| SS spirit storage tanks and flameproof pumps | Holds spirit under excise lock and transfers it | 35.72 |
| SS 316 blending vessels | Mixes spirit and additives in a closed tank | 16.24 |
| Cartridge filtration system | Clears particles and haze | 9.74 |
| Automatic bottle rinsing machine | Cleans bottles before filling | 9.74 |
| Flameproof volumetric filling machine | Fills bottles and cans by volume | 29.22 |
| Screw and ROPP capping machine | Seals packs with tamper-evident caps | 12.99 |
| Labelling and batch coding machine | Applies labels, batch and expiry codes | 16.24 |
| Quality control laboratory | Tests strength, methanol and gravity | 12.99 |
| Fire protection and flameproof electricals | Foam and hydrant system, Ex-rated wiring | 19.48 |
| Total plant and machinery | 162.36 | |

Who buys surgical spirit in India?
Hospitals use surgical spirit to clean skin before injections, to prevent bed sores in long-stay patients and to wipe instruments and surfaces. Retail demand comes through pharmacies, mostly in 100 ml and 400 ml bottles, while hospitals and tenders prefer 500 ml bottles and 5 litre cans.
- Government hospitals and medical colleges, through tenders
- Private hospitals and nursing homes
- Pharmaceutical distributors and stockists
- Retail pharmacies and online pharmacy platforms
- Clinics, dental practices and diagnostic labs
Where a new unit wins: spirit moves between states only with permits and passes, and finished packs are bulky for their value. A licensed unit close to its hospitals and distributors can usually deliver faster and cheaper than a distant brand.
Licences needed to manufacture surgical spirit
Surgical spirit is both a drug and an alcohol product, so two regulators license the unit: the state drug controller and the state excise department. Plan for both before you order machinery.
| Drug manufacturing licence | Form 25 from the State Licensing Authority under the Drugs and Cosmetics Rules, 1945, applied for in Form 24; granted after inspection, with the competent technical staff named on the licence |
|---|---|
| Good manufacturing practice | Revised Schedule M; the extended deadline for small and medium makers with turnover up to ₹250 crore ran to 31 December 2025 |
| Excise licence and spirit permit | State licence to make medicinal preparations containing alcohol, plus permits to buy and hold spirit. In Delhi, for example, these are the M&TP licences L-1 (in bond) and L-2 (outside bond) |
| Raw material standards | BIS-marked methanol under the IS 517 Quality Control Order; IS 4117:2024 is the standard for alcohol denaturants |
| Pollution, fire and factory | State Pollution Control Board consent to establish and operate, fire NOC for flammable liquid storage, and a factory licence where thresholds apply |
| Business registration | Udyam (MSME) registration and GST registration |
Licence names, fees and permit conditions differ by state. Confirm them with your state excise department and drug controller.
Mistakes that weaken a surgical spirit project report
- Pricing sales at MRP
Retail prices on 100 ml bottles are many times the ex-factory rate that distributors and tenders pay, so revenue and DSCR come out far too high.
- No allowance for spirit losses
Some spirit evaporates in transfer, blending and filling, and the excise register must still balance. Saleable output has to allow for it.
- Sales starting before licences
Nothing can be sold until both the drug licence and the excise permit are in hand. The schedule needs a realistic gap for inspections.
- Too little working capital for tender buyers
Trade receivables are the biggest current asset in the sample, ₹166.65 lakh in Year 1. Slow-paying hospitals squeeze a small cash credit limit.
- Leaving out fire safety
Flameproof wiring, foam protection and a separate spirit store are licence conditions, not extras. The sample puts ₹19.48 lakh on them.

Surgical Spirit project report: frequently asked questions
How much does it cost to start a surgical spirit manufacturing unit?
A small blending and filling unit of 500–1,000 litres per day needs roughly ₹60–120 lakh, and a 1,500–2,500 litre unit about ₹1.5–3 crore, including working capital margin. Our sample report models a 3,200 litres per day plant at ₹425.58 lakh. These are indicative figures; your own report works out the cost for your capacity, pack sizes and state.
What is surgical spirit made of?
Surgical spirit is mostly industrial methylated spirit, which is ethanol denatured with a little methanol. Small amounts of castor oil, diethyl phthalate and methyl salicylate are added so the liquid stays on the skin, cannot be drunk and has its familiar smell. Indian packs are labelled to the Indian Pharmacopoeia (IP) and sold for external use only.
Which licences are needed to make surgical spirit in India?
You need two core licences. The first is a drug manufacturing licence in Form 25 from the State Licensing Authority, applied for in Form 24. The second is a state excise licence and permit to buy and use spirit. Add pollution board consent, a fire NOC and the usual Udyam, GST and factory registrations. Licence names and fees differ from state to state.
Is surgical spirit the same as methylated spirit or rubbing alcohol?
Not quite. Methylated spirit is ethanol made undrinkable with methanol and other denaturants, and is used for cleaning and fuel. Surgical spirit starts from methylated spirit but adds castor oil, diethyl phthalate and methyl salicylate for use on skin. Rubbing alcohol is a looser term that can mean an ethanol or isopropyl alcohol product.
Is surgical spirit manufacturing profitable?
In our illustrative sample, a 3,200 litres per day unit earns a net profit of ₹103.40 lakh on sales of ₹1,182.72 lakh in Year 3. Return on investment is 24.3% and break-even comes at 44.2% of capacity. Real profit depends mostly on what you pay for rectified spirit, your pack mix and how quickly hospitals and tenders pay.
Can I get a PMEGP or bank loan for a surgical spirit unit?
PMEGP covers manufacturing projects up to ₹50 lakh, so it suits only a small filling unit. Larger plants usually take an MSME term loan with a cash credit limit, and CGTMSE can guarantee loans to eligible micro and small units up to ₹10 crore. Banks will ask for the drug and excise licences, or proof they have been applied for.
Is the sample report free and can I use it for a loan?
The sample is free to view and download. It is an illustrative report that shows the format and depth of an NPCS report. Its figures are representative and not based on a real project, so it should not be submitted to a bank. For a loan, NPCS prepares a report for your own capacity, location and loan scheme.
