Healthcare and Medical Businesses

Health is a primary human right and has been accorded due importance by the Constitution through Article 21.Though Article 21 stresses upon state governments to safeguard the health and nutritional well being of the people, the central government also plays an active role in the sector. Recognizing the critical role played by the Health Industry, the industry has been conferred with the infrastructure status under section 10(23G) of the Income Act.

The healthcare sector is one of the most challenging and fastest growing sectors in India. Revenues from the healthcare sector account for 5.2 per cent of the GDP, making it the third largest growth segment in India.

The Indian Health sector consists of

-Medical care providers like physicians, specialist clinics, nursing homes, hospitals.

-Diagnostic service centers and pathology laboratories.

-Medical equipment manufacturers.

-Contract research organizations (CRO's), pharmaceutical manufacturers

-Third party support service providers (catering, laundry)

The healthcare industry in the country, which comprises hospital and allied sectors, is projected to grow 23 per cent per annum. According to McKinsey & Co. a leading industrial and management consulting organization, the Indian healthcare sector, including pharmaceutical, diagnostics and hospital services, is expected to more than double its revenues to Rs 2000 billion by 2010. Expenditure on healthcare services, including diagnostics, hospital occupancy and outpatient consulting, the largest component of this spend is expected to grow more than 125% to Rs 1560 billion by 2012 from Rs 690 billion now.

The sector has registered a growth of 9.3 per cent between 2000-2009, comparable to the sectoral growth rate of other emerging economies such as China, Brazil and Mexico. According to the report, the growth in the sector would be driven by healthcare facilities, private and public sector, medical diagnostic and pathology labs and the medical insurance sector.

Healthcare facilities, inclusive of public and private hospitals, the core sector, around which the healthcare sector is centered, would continue to contribute over 70 per cent of the total sector and touch a figure of US$ 54.7 billion by 2012. Adds a FICCI-Ernst and Young report, India needs an investment of US$ 14.4 billion in the healthcare sector by 2025, to increase its bed density to at least two per thousand populations.

Technological advances achieved by medicare globally in the recent years have been phenomenal. The Indian scenario has not remained immune to these changes. While IT (information technology) has come to the aid of the breakthroughs, the progress recorded in the medicare area is as impressive as it is in the IT sector itself. The changes are in concepts, forms and content, as well as applications. These are both, quantitative and qualitative. The transformation is pervasive and has penetrated almost all specialities, from diagnostics to physiotherapy, from cardiology to oncology, from non-invasive surgery to transplants. In India, the emergence of private medicare services, especially through commercialization and corporatization, has contributed to the transformation. The rapid commercialization of the medical practices with the establishment of multi-million rupee hospitals, nursing homes and diagnostic centres, specialized and general, the demand has registered a very high growth rate in the recent years.

Medical sector in India got tax exemptions in the manufacturing of its devices from Union Budget 2010-11 along with the introduction of excellent initiatives towards the development of the sector. The FM has proposed to extend the tax exemption on medical apparatus and devices and concessional tariff available to certified government hospitals. Moreover, the producers of orthopaedic implants have been relieved from import tax.

The incentives proposed by the FM are expected to trigger the expansion of already fast developing medical apparatus and machinery sector in India. By 2010 the medical apparatus and machinery industry is estimated to reach USD 1.8 billion and is projected to expand at a rate of 23% on annual basis as per the NIPER report.

This time the focus of the budget was on rural healthcare, with the fund allocations rising to a whopping 22,300 crores (Rs 223 billion/$4.82 billion) from 19,534 crores during the previous fiscal year. This escalation is in keeping with the evolving needs of the growing healthcare industry of the country. Relaxation of FDI norms may see more international players coming in to India in the healthcare sector. Added to it, rationalization of duties on medical equipment can make imports cheaper and can significantly lower healthcare costs in the country in the coming years.

The government, along with participation from the private sector, is planning to invest US$ 1 billion to US$ 2 billion in an effort to make India one of the top five global pharmaceutical innovation hubs by 2020. The sector has been attracting huge investments from domestic players as well as financial investors and private equity (PE) firms. The Indian market is expanding in all directions as a result of better affordability, greater health consciousness and expanding medical service institutions.

Right hand washing and drying methods stop the chain of transmission of deadly pathogens (from the contaminated surface/site) form hands to other parts of the body. Hand sanitization is the preeminent aid in preventing nosocomial infections caused by different opportunistic microorganisms and to get this, the use of hand sanitizer becomes must in recent circumstances. Herbal hand sanitizer incorporating the leaves extracts of Ocimumcanctum Linn.(Tulsi)and Eucalyptus globules (Nilgiri), the well-known herbal combination with multidimensional activities; and to evaluate their respective antimicrobial efficacy and safety of hands. Herbal hand sanitizer was prepared using leaves extracts of Ocimum sanctum, Eugenia caryophyllus and Cymbopogon flexuous. The antibiotic sensitivity test of the prepared herbal hand sanitizer against skin pathogens was checked using disc diffusion method and results were compared with commercially available synthetic hand sanitizer. The herbal hand sanitizer gives larger inhibition zone than the commercially available synthetic hand sanitizer against Staphylococcus aureus and Pseudomonas aeruginosa. The efficacy of herbal hand sanitizer was evaluated using microorganism suspensions; which revealed that the herbal hand sanitizer is efficient in reducing higher number of microorganism from the hands as compared to commercial synthetic hand sanitizer. Thus, owing to higher antimicrobial activity and efficacy these herbal extracts can be used in the preparation of herbal hand sanitizers on commercial scale. In the absence of a vaccine or effective antiviral drugs, hand hygiene is a mainstay of efforts to prevent the spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the virus that causes COVID-19. The sanitiser contains natural extracted oils, which increase the antiseptic properties and act against bacterial contamination. It also provides cooling and natural fragrance along with moisturizing the skin upon use. India hand sanitizer market is projected to surpass $ 43 million by 2025. Growth of hand sanitizer market in India can be attributed to rising awareness about healthy lifestyle & wellness, shifting consumer preference towards convenient hygiene products and rising disposable income. Moreover, the strong marketing activities by leading brands, in addition to huge endorsements, are some other drivers of hand sanitizer market in India. Moreover, the COVID-19 outbreak has boosted demand for sanitizers like never before across the diverse end user segments. The rising healthcare investments for providing a high-end and safer medical environment for healthcare professionals by preventing hospital-acquired infections (HAIs) have augmented the demand for hand sanitizers in the medical sector. Furthermore, as a result of the COVID-19 pandemic in the country, the demand of hand sanitizers has increased exponentially since the start of 2020. This has led to the widening demand-supply gap and highly inflated product prices. Owing to this, several regional manufacturers are expanding their production capacities in compliance with statutory permissions by the government for maximizing the production of hand sanitizers to meet its robust demand. Besides this, the Indian government has also issued production licenses to several distilleries and new manufacturers to produce hand sanitizers in bulk for adequate supply in the country. India’s workforce is the second largest in the world after China’s. While China’s demographic dividend is expected to start tapering off by 2015, India will continue to enjoy it till 2040. However, India’s formally skilled workforce is approximately 2% - which is dismally low compared to China (47%), Japan (80%) or South Korea (96%).To leverage our demographic dividend more substantially and meaningfully, the Government launched the “Skill India” campaign along with “Make in India”. In this brief, we look at the Skill Development ecosystem in India - the need for Skill Development, initiatives taken by the Government and schemes introduced for skill government by the present government. Alongside the daunting challenge of skilling a lot of youth coming into workforce every month, India also faces a large challenge of evolving a skill development system that can equip the workforce adequately to fulfill the requirements of the business. The workforce has to be trained across four levels, from the high finish specialized skills for ‘White Collar’ jobs to the low-level skills of the ‘Rust Collar’ jobs. Moreover, these skills have to be adequately linked to the on the market job opportunities. Several factors have inhibited the skill development eco-system in India to scale up to the desired levels. The skill development system in India is plagued with multiple issues related to awareness, perception, cost, quality and scale. Even the course curricula often are outdated, redundant and non-standardized. Additionally, the lack of industry-faculty interaction on course curricula leads to irrelevant training modules. The availability of good quality trainers is also a key concern. The quality of trainers is affected due to limited efforts towards re-training and skill improvement of trainers. There is a lack of focus on development of trainers with a clear career path which can make this an aspirational career choice and can ensure regular adequate supply of good-quality trainers in every sector. Active Pharma Ingredients (API) (Cephalexin, Ampicillin Trihydrate, Ibuprofen and Paracetamol)

The Active Pharmaceutical Ingredient (API) is the part of any drug that produces the intended effects. Some drugs, such as combination therapies, have multiple active ingredients to treat different symptoms or act in different ways. Production of APIs has traditionally been done by the pharmaceutical companies themselves in their home countries. But in recent years many corporations have opted to send manufacturing overseas to cut costs. This has caused significant changes to how these drugs are regulated, with more rigorous guidelines and inspections put into place. Cefalexin, is an antibiotic that can treat a number of bacterial infections. It kills gram-positive and some gram-negative bacteria by disrupting the growth of the bacterial cell wall. Cefalexin is a beta-lactam antibiotic within the class of first-generation cephalosporins. It works similarly to other agents within this class, including intravenous cefazolin, but can be taken by mouth. Cefalexin can be used in those who have mild or moderate allergies to penicillin. However, it is not recommended in those with severe penicillin allergies. Ampicillin is an antibiotic used to prevent and treat a number of bacterial infections, such as respiratory tract infections, urinary tract infections, meningitis, salmonellosis, and endocarditis. It may also be used to prevent group B streptococcal infection in newborns. It is used by mouth, by injection into a muscle, or intravenously. Like all antibiotics, it is not useful for the treatment of viral infections. Ampicillin is used to treat infections by many Gram-positive and Gram-negative bacteria. It was the first "broad spectrum" penicillin with activity against Gram-positive bacteria, including Streptococcus pneumoniae, Streptococcus pyogenes, some isolates of Staphylococcus aureus (but not penicillin-resistant or methicillin-resistant strains), Trueperella, and some Enterococcus. Paracetamol (acetaminophen) is a pain reliever and a fever reducer. The exact mechanism of action of is not known. Paracetamol is used to treat many conditions such as headache, muscle aches, arthritis, backache, toothaches, colds, and fevers. It relieves pain in mild arthritis but has no effect on the underlying inflammation and swelling of the joint. Paracetamol is generally safe at recommended doses. The recommended maximum daily dose for an adult is three to four grams. Higher doses may lead to toxicity, including liver failure. Serious skin rashes may rarely occur. It appears to be safe during pregnancy and when breastfeeding. In those with liver disease, it may still be used, but in lower doses. It is classified as a mild analgesic. It does not have significant anti-inflammatory activity. How it works is not entirely clear. Ibuprofen is a medication in the nonsteroidal anti-inflammatory drug (NSAID) class that is used for treating pain, fever, and inflammation. This includes painful menstrual periods, migraines, and rheumatoid arthritis. It may also be used to close a patent ductus arteriosus in a premature baby. It can be used by mouth or intravenously. It typically begins working within an hour India's reliance on pharma ingredient imports has risen over the past few decades due to the higher cost of domestic production, with the price gap reaching as much as 20%-30%, particularly for energy-intensive fermentation-based ingredients used in anti-infectives. Import dependence is more than 90% for some life-saving drugs, including penicillin and ciprofloxacin. India is the seventh largest country in the world and has the second highest population. It has a parliamentary democratic form of government and has abundant natural resources and sufficient oil reserves. The country has a huge skilled, English-speaking, and inexpensive labor force. Its young population and current economic policies have made it one of the largest recipients of FDI in the world. Asia Pacific is expected to be the fastest-growing market over the forecast period. Owing to the availability of affordable labor, major companies in the market are setting up API manufacturing plants in developing countries such as China and India. The global demand of APIs include ageing population, rising expenditures on healthcare, increasing prevalence of lifestyle diseases, etc. Looking forward, the market value is projected to reach US$ 258.8 Billion by 2025, exhibiting a CAGR of 5.6% during 2020-2025. The API market is competitive in nature and is becoming increasingly competitive. Consequently, manufacturers are required to enhance products in order to gain advantage over previously marketed products. The emergence of COVID-19 has brought the world to a standstill. We perceive that this health crisis has brought an unprecedented impact on businesses across industries. However, this too shall pass. Rising support from governments and several companies will help within the fight against this highly contagious disease. There are some industries that are struggling and some are thriving. Overall, almost each sector is anticipated to be impacted by the pandemic. The government also notified a scheme to promote bulk drug parks. For selected parks, financial assistance to the tune of 70 per cent of the project cost of common infrastructure facilities will be provided. In the case of Northeast states and hilly states (Himachal Pradesh, Uttarakhand, Union Territory of Jammu & Kashmir, and Union Territory of Ladakh), financial assistance will be 90 per cent of the project cost. The maximum assistance under the scheme for one bulk Drug Park will be limited to Rs 1,000 crore. The total financial outlay of the scheme is Rs 3,000 crore. Role of Government towards API The coronavirus outbreak disrupting supply of active pharmaceutical ingredients (APIs) and medical devices from China to India, the government has come out with four schemes worth Rs 13,760 crore to encourage manufacturing of bulk drugs and medical devices in the country and their exports. On March 21, the Union Cabinet under the chairmanship of Prime Minister Narendra Modi had approved an expenditure of Rs. 9,940 crore and Rs. 3,820 crore for APIs and medical devices, respectively. The Cabinet also approved a scheme on promotion of bulk drug parks for financing common infrastructure facilities in three bulk drug parks with financial implication of Rs. 3,000 crore for next five years. The government will give grants-in-aid to states with a maximum limit of Rs. 1,000 crore per bulk Drug Park. Parks will have common facilities such as solvent recovery plant, distillation plant, power and steam units, common effluent treatment plant etc. The government further approved production linked incentive (PLI) scheme for promotion of domestic manufacturing of critical KSMs/drug intermediates and APIs in the country with financial implications of Rs. 6,940 crore for next eight years. Financial incentive will be given to eligible manufacturers of identified 53 critical bulk drugs on their incremental sales over the base year (2019-20) for a period of 6 years. Yellow peas are the source of the pea protein isolate and concentrate. These goods offer a plant-based substitute for animal-based proteins like whey and casein that is also suitable for vegans. Having no fats, carbohydrates, or fibre, pea protein isolate is a highly refined form of pea protein. Up to 90% of this product is made up of protein. On the other hand, pea protein concentrate is less processed and includes some fibre and carbs. It contains between 60 and 80 percent protein. People with food sensitivities or allergies, especially those who are lactose intolerant, should use pea protein isolate or concentrate. As opposed to animal-based proteins, it uses less water and land, making it a more sustainable option. Pea protein concentrate and isolate have gained popularity among athletes and fitness enthusiasts recently. This is because pea protein contains significant amounts of amino acids, which are crucial for muscle growth and recuperation. Advantages The nutritional value, environmental effect, and adaptability of pea protein isolate and concentrate are all advantages. Some of the primary advantages are as follows: High Nutritional Value: Pea protein concentrate and isolate have a high protein content of up to 90%. Additionally, they are abundant in essential amino acids, which are important for muscle growth and repair. Pea protein is a great substitute for folks who have dairy or soy allergies because it is easily absorbed and hypoallergenic. Environmentally Friendly and Sustainable: An environmentally friendly substitute for animal-based proteins is pea protein. Peas have a lower carbon footprint than protein sources derived from animals because they require less water and fertiliser. In comparison to the production of animal-based proteins, pea protein generates less waste and emits fewer greenhouse gases. Flexible Ingredient: Protein bars, snacks, meat substitutes, and sports nutrition items can all be made with pea protein isolate and concentrate. Additionally, they can be utilised as a functional component in a variety of processes, including emulsification, foaming, and gelling. They can also lower cholesterol levels. They also include a lot of antioxidants, which can help prevent chronic illnesses including cancer, diabetes, and heart disease. Economic Advantages: Growing consumer demand for plant-based proteins opens up business prospects for food industry entrepreneurs. The demand for plant-based protein is anticipated to increase globally, giving entrepreneurs the chance to create cutting-edge goods and cash in on this trend. Market Outlook for India The forecast for the pea protein isolate and concentrate industry in India is positive. In India, there is an increasing need for plant-based protein sources, and pea protein has become recognised as an excellent substitute for conventional animal-based protein sources. In the upcoming years, India's market for plant-based protein is anticipated to expand significantly. The market for plant-based proteins is expanding in India as a result of rising consumer demand for vegan and vegetarian goods as well as rising public awareness of the health advantages of plant-based diets. Additionally, the demand for pea protein isolate and concentrate in India is likely to increase due to the Indian government's emphasis on encouraging the use of plant-based proteins as a means of lowering the nation's carbon footprint. These trends can be used by businesspeople to get into the industry and profit from India's rising need for plant-based protein. Outlook for the World Market The size of the worldwide pea protein market was USD 416.39 million in 2020 and is anticipated to increase by 12.0% CAGR from USD 464.60 million in 2021 to USD 1,026.12 million by 2028. The pea, a leguminous plant, contains a significant quantity of protein (20–30%) in its seeds. It mostly exists as globulins, which are the primary ingredients of products made from pea protein isolate (PPI). Wet-milling and dry-milling techniques can be used to create pea protein, with a protein concentration ranging from 48% to 90%. The important functional characteristics of PPI and concentrates include nutritional advantages, oil-binding capacity, water-binding capacity, foam stability, foam expansion, whip ability, emulsion stability, gelatin, and emulsion ability ratio. Over the forecast period, the regional market is anticipated to be driven by rising demand for gluten-free products, rising worries about Cardiovascular Diseases (CVDs) brought on by red meat consumption, and strong growth in the sports nutrition sector in North America. Due to the rising popularity of energy bars, cold cereals, and snacks, the protein (pea) market in North America is anticipated to expand at a high rate over the forecast period. The market demand for protein components is increased by new product debuts by businesses like Cargill and Mead Johnson that focus on consumer demands for beverages with reduced saturated fat and no cholesterol. Conclusion The sports nutrition market is also fueling the boom in the pea protein isolate and concentrate industry, which is a result of the expanding health and fitness trend. In the food and beverage sector, pea protein isolate and concentrate have a promising future because they provide customers with a high-quality, reasonably priced, and adaptable plant-based protein alternative without compromising on taste or quality.
